Who qualifies for Home Care?

Individuals who are:

• - Home Bound
• - Under the care of a physician and medically unstable
• - Recovering from the hospital due to surgery, accident or illness
• - In need for Skilled Nursing, Physical, Occupational or speech Therapy; Medical
• - Social Workers or Certified Home Health Aides

Any patient, family, friend, hospital or physician can request home care services.
